病程自第二周開始為持續高熱，多為稽留熱型，經歷約10 ~14天。此期中毒症狀多明顯，可表現表情淡漠，反應遲鈍，舌質紅，苔黃膩，腹痛、腹脹、腹瀉，肝脾腫大，少數年長兒可呈現緩脈，極少數病兒胸腹部可見少量淡紅色丘疹，也稱玫瑰疹，2～3日即消失。

心源性哮喘：引起左心衰竭的原發病，如冠心病、風心病等，多見中年以上，吸氣性呼吸困難為主，坐起後可減輕，左心室增大，出現舒張期奔馬律，兩肺底有濕羅音，X線檢查肺淤血征，心影擴大。

般應住院觀察，病人絕對卧床休息，至少四周。保持安靜，盡量不要多搬動，大小便也不可起床。應用中、西葯物止血，控制腦水腫。頭痛、煩燥、給予止痛、鎮靜葯物，如異丙嗪、可待因、地西泮等，嘔吐者，給予止吐葯物。如果出現劇烈的頭痛，頻繁的惡心嘔吐，脈率和呼吸變慢，甚至於昏迷，而上述保守法無效時，即應考慮手術治療。

3) 試述重型肝炎的治療。
患者應卧床休息，實施重症監護，密切觀察病情，防止醫院感染。每日應給予以碳水化合物為主的營養支持治療，以減少脂肪和蛋白質的分解。補液量約1500～2000ml/d，注意出入量的平衡，尿量多時可適當增加補液量。注意維持電解質及酸鹼平衡。供給足量的白蛋白，盡可能減少飲食中的蛋白質，以控制腸內氨的來源，維持正氮平衡、血容量和膠體滲透壓，預防腦水腫和腹水的發生。補充足量維生素B、C及K。輸注新鮮血漿、白蛋白或免疫球蛋白以加強支持治療。禁用對肝、腎有損害的葯物。
3) 簡述腦血栓與腦出血的鑒別。
腦栓塞的栓子是活動的，腦血栓的是靜止的。
腦栓塞是指因異常的固態、液態、氣態物體（被稱作栓子）沿血循環進入腦動脈系統，引起動脈管腔閉塞，導致該動脈供血區局部腦組織的壞死，臨床上表現為偏癱、偏身麻木、講話不清等突然發生的局源性神經功能缺損症狀。
腦血栓形成是在腦動脈粥樣硬化和斑塊基礎上，常在睡眠或安靜休息時由於血流緩慢、血壓偏低等因素的促使下，血液的有形成分附著在動脈的內膜形成血栓，稱之為腦血栓。臨床上以偏癱為主要表現。
7、試述甲狀腺危象的臨床表現。
又稱甲亢危象，是甲狀腺毒症急性加重的一個綜合征，發生原因可能與循環中的甲狀腺激素水平增高有關。多發生於較重甲亢未予治療或治療不充分的患者。常見誘因有感染、手術、精神刺激等，臨床表現為高熱、大汗、心動過速、煩躁、焦慮不安、譫妄、惡心、嘔吐、腹瀉，嚴重患者可有心衰，休克和昏迷等。
8) 試述消化性潰瘍並上消化道出血的處理。
1．一般處理卧床休息，注意保暖，必要時吸氧，在休克狀態或胃脹滿、惡心情況下應禁食。對小量出血、無嘔吐的患者，可進流質或半流質易消化飲食。2．補充血容、抗休克對於大出血的患者首先應補充有效循環血容量，最佳的辦法是短期內快速輸血。要求在1~3h內將丟失量的1/4~1/3輸入。3．局部用葯止血這是最常用的應急處理方法。對出血嚴重者，可用去甲腎上腺素4~8mg加入100ml生理鹽水（或冰鹽水）中，口服或經胃管注入，此法可使胃血管暫時性收縮，從而達到止血目的，10~15min可重復一次。4．全身用葯止血胃內酸性環境可抑制凝血過程和血小板的功能，依賴酸性環境的胃蛋白酶及溶纖維素可對血凝塊產生溶解作用，故應用抑酸葯物使胃液pH值保持在4~5以上時，止血效果較好，pH值為7時最理想。5．內鏡下止血內鏡下止血方法很多，其中局部噴灑止血葯、局部注射治療及溫熱止血為目前最主要的方法，6．介入治療常用的方法是經選擇性動脈造影導管灌注葯物及栓塞治療最常用的葯物是血管升壓素，血管造影發現出血部位後，盡量把導管頭端插入出血所在部位之動脈分支，經造影導管灌注血管升壓素。如果導管不能被送入出血所在分支時，也可根據出血部位，把導管頭端置於腹腔動脈或腸系膜上動脈。7．手術治療
9、簡述肝硬化失代償期臨床表現。
肝硬化失代償期就是肝硬化晚期的症狀表現，一般指肝硬化發展到一定程度，超出肝功能的代償能力，臨床有明顯的病理變化。主要表現為肝功能損害，有門脈高壓，脾大，腹水、肝性腦病或上消化道出血，這個時期叫做肝硬化失代償期。

（1）年齡：1型糖尿病大多數為40歲以下發病，20歲以下的青少年及兒童絕大多數為1型糖尿病，僅極少數例外；2型糖尿病大多數為40歲以上的中老年人，50歲以上的人患1型糖尿病很少。總之，年齡越小，越容易是1型糖尿病；年齡越大，越容易是2型糖尿病。 （2）起病時體重：發生糖尿病時明顯超重或肥胖者大多數為2型糖尿病，肥胖越明顯，越易患2型糖尿病；1型糖尿病人在起病前體重多屬正常或偏低。無論是1型或2型糖尿病，在發病之後體重均可有不同程度降低，而1型糖尿病往往有明顯消瘦。（3）臨床症狀：1型糖尿病均有明顯的臨床症狀如多飲、多尿、多食等，即「三多」，而2型糖尿病常無典型的「三多」症狀。為數不少的2型糖尿病人由於臨床症狀不明顯，常常難以確定何時起病，有的只是在檢查血糖後才知道自己患了糖尿病。1型糖尿病人由於臨床症狀比較突出，故常能確切地指出自己的起病時間。（4）急慢性並發症：1型與2型糖尿病均可發生各種急慢性並發症，但在並發症的類型上有些差別。就急性並發症而言，1型糖尿病容易發生酮症酸中毒，2型糖尿病較少發生酮症酸中毒，但年齡較大者易發生非酮症高滲性昏迷。就慢性並發症而言，1型糖尿病容易並發眼底視網膜病變、腎臟病變和神經病變，發生心、腦、腎或肢體血管動脈硬化性病變則不多見，而2型糖尿病除可發生與1型糖尿病相同的眼底視網膜病變、腎臟病變和神經病變外，心、腦、腎血管動脈硬化性病變的發生率較高，合並高血壓也十分常見。因此2型糖尿病人發生冠心病及腦血管意外的機會遠遠超過1型糖尿病人，這是一個十分明顯的不同點。（5）臨床治療：1型糖尿病只有注射胰島素才可控制高血糖，穩定病情，口服降糖葯一般無效。2型糖尿病通過合理的飲食控制和適當的口服降糖葯治療，便可獲得一定的效果，當然當口服降糖葯治療失敗、胰島B細胞功能趨於衰竭或出現嚴重的急慢性並發症時，也是胰島素的適應症。（6）對於那些通過臨床表現很難判斷是那種類型糖尿病的患者，常常需要進一步的檢查。這些檢查包括：空腹及餐後2小時胰島素或C肽檢查：可以了解患者體內胰島素是絕對缺乏還是相對缺乏。（7）各種免疫抗體的檢查：如GAD抗體、ICA抗體等，這些抗體檢查可以了解患者的糖尿病是否於免疫有關。
12)強心甙中毒的臨床表現如何？
（1）胃腸道反應：常見，也出現較早；表現為厭食、惡心、嘔吐及腹瀉；其中食慾減退往往是中毒的最早表現。上述表現與強心苷用量不足、心功能不全未能糾正或胃腸瘀血時的表現酷似，應注意鑒別。
（2）神經系統表現：包括中樞神經系統症狀如頭痛、頭暈、疲乏、不適、失眠及譫妄等；以及視覺障礙如色視（黃視症或綠視症）和視力模糊。色視為重要的中毒先兆，可能與強心苷分布在視網膜中或與電解質紊亂有關。
反跳性高血壓時可給予酚妥拉明或酚苄明。為了拮抗普萘洛爾的β阻滯作用，所需異丙腎上腺素或去甲腎上腺素的量有時相當大；應在監測心率、血壓和心電圖的前提下逐漸加大劑量，直至中毒症狀好轉、消失。
（3）心臟毒性：為最危險的中毒症狀，可誘發各種類型的心律失常。其中較常見且具特徵性的心律失常有室性早搏二聯、三聯律，多源性室性早搏，房室交界性心律特別是交界性″心動過速，心房纖顫合並房室傳導阻滯，室性心動過速及所謂的雙向性心動過速等。意外超量中毒時主要發生傳導紊亂，以竇房傳導阻滯或房室傳導阻滯最常見。因此在用葯過程中，如發生心率異常增快或減慢，發生心律改變，無論是整齊轉為不齊或由不齊變為整齊，均需警惕強心苷中毒，應立即監測心電圖。

Ⅱ Ⅲ AVF導聯ST段明顯太高（就是高尖的尖尖後面的部分）
他表示的就是心臟的下壁，
所以說下壁梗死是確定的，

為什麼說後壁也有問題呢？
因為V2導聯R波稍高，ST段壓低，
正常情況下是不應該出現的，
這種情況高度提示後壁梗死，
並且，下壁梗死很容易合並後壁梗死，
這在心內科醫生是個常識性的問題。

治療？
