2、试述伤寒极期的典型临床表现。
病程自第二周开始为持续高热，多为稽留热型，经历约10 ~14天。此期中毒症状多明显，可表现表情淡漠，反应迟钝，舌质红，苔黄腻，腹痛、腹胀、腹泻，肝脾肿大，少数年长儿可呈现缓脉，极少数病儿胸腹部可见少量淡红色丘疹，也称玫瑰疹，2～3日即消失。

3) 简述蛛网膜下腔出血的处理。
般应住院观察，病人绝对卧床休息，至少四周。保持安静，尽量不要多搬动，大小便也不可起床。应用中、西药物止血，控制脑水肿。头痛、烦燥、给予止痛、镇静药物，如异丙嗪、可待因、地西泮等，呕吐者，给予止吐药物。如果出现剧烈的头痛，频繁的恶心呕吐，脉率和呼吸变慢，甚至于昏迷，而上述保守法无效时，即应考虑手术治疗。

3) 试述重型肝炎的治疗。
患者应卧床休息，实施重症监护，密切观察病情，防止医院感染。每日应给予以碳水化合物为主的营养支持治疗，以减少脂肪和蛋白质的分解。补液量约1500～2000ml/d，注意出入量的平衡，尿量多时可适当增加补液量。注意维持电解质及酸碱平衡。供给足量的白蛋白，尽可能减少饮食中的蛋白质，以控制肠内氨的来源，维持正氮平衡、血容量和胶体渗透压，预防脑水肿和腹水的发生。补充足量维生素B、C及K。输注新鲜血浆、白蛋白或免疫球蛋白以加强支持治疗。禁用对肝、肾有损害的药物。
3) 简述脑血栓与脑出血的鉴别。
脑栓塞的栓子是活动的，脑血栓的是静止的。
脑栓塞是指因异常的固态、液态、气态物体（被称作栓子）沿血循环进入脑动脉系统，引起动脉管腔闭塞，导致该动脉供血区局部脑组织的坏死，临床上表现为偏瘫、偏身麻木、讲话不清等突然发生的局源性神经功能缺损症状。
脑血栓形成是在脑动脉粥样硬化和斑块基础上，常在睡眠或安静休息时由于血流缓慢、血压偏低等因素的促使下，血液的有形成分附着在动脉的内膜形成血栓，称之为脑血栓。临床上以偏瘫为主要表现。
7、试述甲状腺危象的临床表现。
又称甲亢危象，是甲状腺毒症急性加重的一个综合征，发生原因可能与循环中的甲状腺激素水平增高有关。多发生于较重甲亢未予治疗或治疗不充分的患者。常见诱因有感染、手术、精神刺激等，临床表现为高热、大汗、心动过速、烦躁、焦虑不安、谵妄、恶心、呕吐、腹泻，严重患者可有心衰，休克和昏迷等。
8) 试述消化性溃疡并上消化道出血的处理。
1．一般处理卧床休息，注意保暖，必要时吸氧，在休克状态或胃胀满、恶心情况下应禁食。对小量出血、无呕吐的患者，可进流质或半流质易消化饮食。2．补充血容、抗休克对于大出血的患者首先应补充有效循环血容量，最佳的办法是短期内快速输血。要求在1~3h内将丢失量的1/4~1/3输入。3．局部用药止血这是最常用的应急处理方法。对出血严重者，可用去甲肾上腺素4~8mg加入100ml生理盐水（或冰盐水）中，口服或经胃管注入，此法可使胃血管暂时性收缩，从而达到止血目的，10~15min可重复一次。4．全身用药止血胃内酸性环境可抑制凝血过程和血小板的功能，依赖酸性环境的胃蛋白酶及溶纤维素可对血凝块产生溶解作用，故应用抑酸药物使胃液pH值保持在4~5以上时，止血效果较好，pH值为7时最理想。5．内镜下止血内镜下止血方法很多，其中局部喷洒止血药、局部注射治疗及温热止血为目前最主要的方法，6．介入治疗常用的方法是经选择性动脉造影导管灌注药物及栓塞治疗最常用的药物是血管升压素，血管造影发现出血部位后，尽量把导管头端插入出血所在部位之动脉分支，经造影导管灌注血管升压素。如果导管不能被送入出血所在分支时，也可根据出血部位，把导管头端置于腹腔动脉或肠系膜上动脉。7．手术治疗

肝硬化失代偿期就是肝硬化晚期的症状表现，一般指肝硬化发展到一定程度，超出肝功能的代偿能力，临床有明显的病理变化。主要表现为肝功能损害，有门脉高压，脾大，腹水、肝性脑病或上消化道出血，这个时期叫做肝硬化失代偿期。

（1）年龄：1型糖尿病大多数为40岁以下发病，20岁以下的青少年及儿童绝大多数为1型糖尿病，仅极少数例外；2型糖尿病大多数为40岁以上的中老年人，50岁以上的人患1型糖尿病很少。总之，年龄越小，越容易是1型糖尿病；年龄越大，越容易是2型糖尿病。 （2）起病时体重：发生糖尿病时明显超重或肥胖者大多数为2型糖尿病，肥胖越明显，越易患2型糖尿病；1型糖尿病人在起病前体重多属正常或偏低。无论是1型或2型糖尿病，在发病之后体重均可有不同程度降低，而1型糖尿病往往有明显消瘦。（3）临床症状：1型糖尿病均有明显的临床症状如多饮、多尿、多食等，即“三多”，而2型糖尿病常无典型的“三多”症状。为数不少的2型糖尿病人由于临床症状不明显，常常难以确定何时起病，有的只是在检查血糖后才知道自己患了糖尿病。1型糖尿病人由于临床症状比较突出，故常能确切地指出自己的起病时间。（4）急慢性并发症：1型与2型糖尿病均可发生各种急慢性并发症，但在并发症的类型上有些差别。就急性并发症而言，1型糖尿病容易发生酮症酸中毒，2型糖尿病较少发生酮症酸中毒，但年龄较大者易发生非酮症高渗性昏迷。就慢性并发症而言，1型糖尿病容易并发眼底视网膜病变、肾脏病变和神经病变，发生心、脑、肾或肢体血管动脉硬化性病变则不多见，而2型糖尿病除可发生与1型糖尿病相同的眼底视网膜病变、肾脏病变和神经病变外，心、脑、肾血管动脉硬化性病变的发生率较高，合并高血压也十分常见。因此2型糖尿病人发生冠心病及脑血管意外的机会远远超过1型糖尿病人，这是一个十分明显的不同点。（5）临床治疗：1型糖尿病只有注射胰岛素才可控制高血糖，稳定病情，口服降糖药一般无效。2型糖尿病通过合理的饮食控制和适当的口服降糖药治疗，便可获得一定的效果，当然当口服降糖药治疗失败、胰岛B细胞功能趋于衰竭或出现严重的急慢性并发症时，也是胰岛素的适应症。（6）对于那些通过临床表现很难判断是那种类型糖尿病的患者，常常需要进一步的检查。这些检查包括：空腹及餐后2小时胰岛素或C肽检查：可以了解患者体内胰岛素是绝对缺乏还是相对缺乏。（7）各种免疫抗体的检查：如GAD抗体、ICA抗体等，这些抗体检查可以了解患者的糖尿病是否于免疫有关。
12)强心甙中毒的临床表现如何？
（1）胃肠道反应：常见，也出现较早；表现为厌食、恶心、呕吐及腹泻；其中食欲减退往往是中毒的最早表现。上述表现与强心苷用量不足、心功能不全未能纠正或胃肠瘀血时的表现酷似，应注意鉴别。
（2）神经系统表现：包括中枢神经系统症状如头痛、头晕、疲乏、不适、失眠及谵妄等；以及视觉障碍如色视（黄视症或绿视症）和视力模糊。色视为重要的中毒先兆，可能与强心苷分布在视网膜中或与电解质紊乱有关。
反跳性高血压时可给予酚妥拉明或酚苄明。为了拮抗普萘洛尔的β阻滞作用，所需异丙肾上腺素或去甲肾上腺素的量有时相当大；应在监测心率、血压和心电图的前提下逐渐加大剂量，直至中毒症状好转、消失。
（3）心脏毒性：为最危险的中毒症状，可诱发各种类型的心律失常。其中较常见且具特征性的心律失常有室性早搏二联、三联律，多源性室性早搏，房室交界性心律特别是交界性″心动过速，心房纤颤合并房室传导阻滞，室性心动过速及所谓的双向性心动过速等。意外超量中毒时主要发生传导紊乱，以窦房传导阻滞或房室传导阻滞最常见。因此在用药过程中，如发生心率异常增快或减慢，发生心律改变，无论是整齐转为不齐或由不齐变为整齐，均需警惕强心苷中毒，应立即监测心电图。

Ⅱ Ⅲ AVF导联ST段明显太高（就是高尖的尖尖后面的部分）
他表示的就是心脏的下壁，
所以说下壁梗死是确定的，

为什么说后壁也有问题呢？
因为V2导联R波稍高，ST段压低，
正常情况下是不应该出现的，
这种情况高度提示后壁梗死，
并且，下壁梗死很容易合并后壁梗死，
这在心内科医生是个常识性的问题。

治疗？
